Shock wave lithotripsy (SWL) is a medical procedure that uses shock waves (SWs) generated extracorporeally to break urinary stones. SWL revolutionized the surgical management for stone diseases and lithotripsy technologies have evolved considerably over the past three decades in many aspects but the success rate of SWL remained stalemated. As the only non-invasive treatment option it is currently used for treating over half the patients, there is a great need to improve this technique.
The author will demonstrate how the change of acoustic coupling from the original ideal water-bath type to “dry” coupling using ultrasound gel in modern lithotripters reduces stone breakage efficiency, and how body wall in the SW-path affects the acoustic field near the target. Random air pockets trapped at the coupling interface between therapy head and patient skin are known to significantly decrease stone breakage. The author designed and carried out in vitro experiments to further determine the effect of size and location of coupling defects. Knowledge of the acoustic field is important for understanding SW action in stone breakage but in vivo measurement is very difficult. The author innovatively adopted an ex vivo approach to best mimic the in vivo environment and to ensure precise acoustic measurement.
Experimental results from both the acoustic coupling and body wall tests will be presented. If time permits, the author would also like to present his doctoral work on Resonant Ultrasound Spectroscopy (RUS) and how he used this accurate elastic characterization technique to study thermoelectric materials at high temperatures.

李广岩博士,现在印第安那大学医学院(Indiana University School of Medicine)从事超声碎石(Shock Wave Lithotripsy)有关的研究工作;此前他在密西西比大学的国家物理声学中心从事共振超声谱方面的博士研究;他在青岛海洋大学还简短地从事了海洋声学方向的硕士研究。
